Service Agreement & Budget *

After submission, your client record will be set up, funding secured, service agreement & consent sent for signature. Telehealth Assessment requires a minimum of 10hrs @ $232.99 Total $2,329.90 within CB Daily Activities budget. If the assessment will be taking place in person, additional non face to face, travel and kms will apply and a quote will be sent.
